For a natural shake all you need is food that tastes good together.
There is a post a few down titled "Healthy Protein or Weight Gain Powder" in which I posted a recipe I used to use. Others have posted their recommendations as well.
Last night I thought of another option, which I believe is all natural. It is a shake-type drink you can get at grocery stores, though I forget what it is called. I'm sure you have seen it in the produce section, though.
I plan on buying one later today, and I'll post the brand and nutrional facts later (it has a lot of calories in it).
You could also look into Carnation Instant Breakfasts. They are a lot of calories and nutrition in a small package. Some of their flavors are pretty tasty too. You could drink them solo, or combine them with a weight gain shake for lost of calories.

Edit: I just checked out the calories in a Carnation Instant Breakfast; it's only 60 caloires - much less than I thought
