Definitely not an expert and not attempting to answer all three questions.
RE:#2 - I have seen a compelling argument made that a rise in insulin is necessary to finalize the metabolic process of building muscle and eliciting GH/Testosterone response post-workout.
Based on this info your dietary habits are 100% correct 99% of the time...just get some quick digesting carbs and protein (BCAA's??) post workout. The critical timeframe seems to be within 30 minutes of completing a workout. Follow your current regimen other than this one minor tweak.
How or why BCAAs play into this I have no idea.
