Yes as Steve said you have to get a diagnosis or suspect in order for them to even follow up..
My insurance covers it all, I pay about $11.00 for a 10 ml bottle that lasts me about 4.5 months.
Needles are cheap, but order online in bulk.
Bluecross/blue shield is what I have and it covers such things.
